Questionnaire Content

This will be the first text that the patient will receive.

Before you start

You need a blood pressure machine to do this. If you do not have one, please contact your GP.

How it works 

1.In the morning, take the first number (SYS).

2. Take the second number (DIA).

3. Make a note of both numbers.

4. Do the same in the afternoon.

5. Repeat for 7 days.

6. At the end of 7 days, we’ll send you a link to submit your readings.

Take the fist number (SYS)

Make a note of the first number on your blood pressure monitor. It’s the one with ‘SYS’ next to it.

If this number is 175 or higher, make an urgent same-day appointment to see your GP.

Take the second number (DIA)

Make a note of the second number on your blood pressure monitor. It’s the one with ‘DIA’ next to it. 

If this number is 115 or higher, make an urgent same-day appointment to see your GP.

Take readings twice a day

Take readings every morning and every afternoon or evening. Do this for 7 days.

It might help to set a reminder on your phone to help you remember.

Write the readings down somewhere safe. At the end of the 7 days we’ll send you a link to submit your readings.

Do not worry if you forget to take readings at any point - just submit what you have.

The following reminder will be sent consecutively every day at any time

Reminders can be sent in the morning, and afternoon but the final SMS will be sent at the end of this sequence

Submit Readings

Once the patient has taken there BP for 7 days they will receive a reminder instruction them to submit their blood pressure readings 

once link is accessed patient will be asked for their date of birth

Confirm your date of birth

Submit your blood pressure readings

If 2 readings in a row are higher than 170 SYS or 115 DIA, please contact your GP practice to ask for an urgent appointment.

Day 1 - morning

Day 1 - afternoon or evening

Day 2 - morning

Day 2 - afternoon or evening

Etc etc screens till 7 days of readings are submitted 


Check and submit your answers

Please check your answers are correct before sending them to your GP practice.

If 2 readings in a row are higher than 170 SYS or 115 DIA, please contact your GP practice to ask for an urgent appointment.

The above screen will show you what you have submitted, the change button will allow you to update any mistakes.
