Coding the PHQ-9 score directly into S1

A technical limitation means we cannot yet code the PHQ-9 score directly into S1. We are working to fix this issue. While we do this, we have configured MJOG to code to S1 that a depression screening has been completed, and free text the score alongside. This issue is not present in EMIS.
